As an independent applicant, the child needs to fill out a separate visa application form. Moreover, the underaged applicants need to show their birth certificate (original + copy) as well as both parents’ consent to the child’s departure (either both parents should come in person to fill in the consents at the Visa Center or their consents should be notarized).
