Notice on submitting the "Health Declaration Form for Visa Application
According
to relevant laws and regulations of the People's Republic of China, starting
from now on, all visa applicants ( except diplomats ) are required to submit
the "Health Declaration Form for Visa Application" (attached), and
the visa application will not be accepted if this form is not submitted.
