The consent of submitting personal data

Dear visa applicants:

    In accordance with the Malaysian government on PDPA Act(Personal Data Protection Act 2010),all the applicants or third party shall submit a copy of the Declaration upon visa application,effective on 17th October 2016. The consent form can be downloaded from the Centre's website at "Downloads"column.Kindly print the form,fill in the required details and sign.
