You may download a blank application form either in PDF or Word version onto your computer and fill in manually. Kindly remind you to print out the completed application form in two pages (A4 size) and sign where appropriate. Please also remember to make an appointment online in advance and submit your application at the Visa Centre on the date of appointment.
